A Commission Report box will come up.
Enter the Start and End dates for the month(s). For example, for a report for October, 2002, enter Start Date 10/01/2002 and End Date 10/31/2002.

Print/Fax (hard copy)
Email (you have to enter an address to email to before choosing this selection)
